What Supporting Documentation do I need to share when setting up a disabled fan account?

We accept any of the following as supporting evidence:

- A statement of higher or middle-rate Disability Living Allowance (DLA) as issued by the Department of Work & Pensions (DWP)
- The award letter for Personal Independence Payment (PIP)
- Receipt of Severe Disablement Allowance or Attendance Allowance
- Blind or Partially Sighted registration certificate 

We may also consider an Access Card or alternative supporting documentation. With non-UK residents supplying equivalent evidence from their home country.

The evidence you provide must show the level of award and the expiry date, like the below examples:

Supporters who wish to book Blue Badge Parking on matchdays, should also send a copy of their blue badge that must show your photograph, the badge number and the expiry date, like the below example:
